Son oif Michael O'Farrell and Ellen J. O'Farrell of Inverell, NSW later of Ipswich, QLD.
Served in the Queensland Police for about a year.
During September 1915 Samuel gave his marital status as widower and named his daughter Irene O'Farrell of Prospect Terrace, Kelvin Grove, QLD as his next of kin. Also father of Daisy O'Farrell
