Son of Rodger O'Sullivan and Bridget O'Sullivan nee Salmon of Richmond, QLD.
On 3 August 1917 he was mention in Divisional Routine Orders.
Commenced return to Australia on 4 December 1918 aboard HT Somerset disembarking on 15 January 1919 at Melbourne for onward travel to Sydney
