Son of John William Ullyatt and Sarah Ellen Ullyatt nee Taylor of Northcote, Victoria. Brother of John Harrold Ullyatt who returned to Austalia on 13 August 1919 having served with the 17th Field Ambulance and 1st Casualty Clearing Station

Troopship HT Medic recalled arriving back in Melbourne on 12 December 1918.
