O'Sullivan's birth Christian Names were Patrick John, however, when he enlisted for World War One he only used his second name.

He had previously served in the RAN, enlisting on 27 May 1913 (Service Number 2967) and his file has a note dated 3 June 1915 that he had enlisted in the AIF. This was four days after his AIF enlistment so it is probable that he purposely used his second name to avoid any complications by being in two services at the same time.

When he re-enlisted for World War Two service in the Army he used his correct name.
