Benjamin Douglas JACK was born on 5th December, 1893 in Carlton, Victoria
His parents were Benjamin James JACK and Sarah DOUGLAS
He enlisted on 24th August, 1914 as a 2nd Lieutenant with the 5th Battalion
He had previous service for 5 years with the Senior Cadets and 2 years as Lieutenant with 46 area Senior Cadets and 14 months as 2nd Lieutenant with the 49th Infantry
He was Killed in Action on 9th April, 1918 - rank Major
He is buried in Gentelles Communal Cemetery in France, Grave Ref. 14
