On 10 June 1959 while living at Murray Street, Colac, Victoria and applying for replacement medals he signed a Statutory Declaration under his full name was Frederick Charles William Splatt and that he served in the WW1 AIF under the name of Frederick Splatt
Address at the time of enlistment was C/- R. donaldson of 'Fairview', Woodfield, Victoria
