'In the vicinity of BELLEWARDE RIDGE between 27th/28th September, 1917, while working under heavy shell fire, this man showed great devotion to duty and exceptional organising capacity in dressing and evacuating a continuous stream of patients and maintaining supplies. Although offered relief, he was anxious to see the work through and voluntarily day and night remained on duty during the whole of this time.'
