Only surviving son of Mr and Mrs W FORMBY, and the grandson of the Late John FORMBY, Mayor of Port Adelaide.

Husband of Ivy Charlotte FORMBY of 1 Gilmore Road, South Henley, SA.

Educated at Kenton Valley and Lockleys Public School he entered the firm of Clarksons Ltd. and was there many years.  His hobby was yachting and he was a member of the Grange Dinghy Club, in the crew of 'Sunny South'.  He enlisted in July 1940.
