The appointment of No. 400366 Flight Sergeant David Crawford Forman (Pilot) to a commission on probation notified in Commonwealth of Australia Gazette, No. 234, dated 4th November, 1943, is amended to take effect from 15th March, 1943, in lieu of 15th April, 1943. His probationary appointment is confirmed and he is promoted to Flying Officer with effect from 15th September, 1943.

Flying-Officer David Crawford Forman, youngest son of Mrs. F M Forman of 10 Fairview-grove East Malvern and the late George Forman, has been killed in air operations over India. F./O. Forman was 23  years of age. He was educated at Malvern Grammar and Scotch College, and was on the staff of the T. and G. Mutual Life Assurance Society Ltd. when he enlisted in August, 1940. After gaining his wings  at Wagga he left for England, via Canada, as a sergeant-pilot in June, 1941, and did an intense navigational course. He then flew to India, and was posted to an R.A.F. squadron, where he had  considerable success as a bomber pilot in operations against the Japanese in the Andaman Islands and on the Burma front.

Airman Killed In Monsoon
FLYING-OFFICER DAVID CRAWFORD FORMAN, of East Malvern, who was awarded the Air Force Cross in the New Year honors list, was killed when his aircraft ran into a monsoon on returning to base  from, Calcutta, R.A.A.F headquarters stated yesterday. Flying-Officer Forman was educated at Malvern Grammar School and Scotch College. He enlisted in August 1940, after gaining his wings at Wagga he left for England, via Canada, as a sergeant-pilot in June, 1941, where he completed an intense navigational course. He then flew to India and was posted to a RAF squadron, where he had considerable success as a bomber pilot in operation against the Japanese in the Andaman Islands and on the Burma front. With more than 400 operational hours to his credit Flying-Officer Forman had also been engaged in transport work over The Hump to China, regarded officially as a most hazardous flight.
