Sergeant Jack Holt 3514 and Leading Aircraftman Thomas Hodge 403052 were killed when Tiger Moth A17-254 crashed on to the Lakes Golf Links in Sydney after it collided in flight with Tiger Moth A17-252 - both aircraft were from Number 4 Elementary Flying Training School based at Mascot airfield

The crew of Tiger Moth A17-252 both survived the incident after being able to crash-land their aircraft - Pilot Officer John Cherry 291880 and John Pettett 403372 (O211029)

The two fatalities were cremated at the Eastern Suburbs Crematorium (Sydney) and they are commemorated on the NSW Cremation Memorial
