Accidentally injured 25/01/1943. Died 26/01/1943. Buried 27/01/1943.
Report of an injury: Lacerated bicep right arm lacerated neck, haematoma of right lymph area cerebral damage. Did not recoover consciesness after the accident and died from cerebral injury at 0730hrs 26/01/1943. Sustained whilst on duty not due to negligence or misconduct. Unknown if anyone else to blame until c of Iheld court of inquiry will be held by high authorty than CO of unit.
Died of injuries at Penrith Hospital. Buried Penrith General Cemetery 27/01/1943. Grave no 7, Section L, R.C portion.

10.02.1943 - Department of the Attorney General and of Justice.
Coronial Inquiry held at Penrith NSW 04/02/1943 found that the deceased on 26/01/1943 at Nepean District hospital died from the effects of injuries accidentally received via laceration of the brain caused through thrown on 25/01/1943 on the Hawkesbury Road North Springwood.

10.02.1943 2 AUST MOTOR BDE GP. Court of Inquiry held at the Gherang 10.02.1943 declared that approx 2 miles north of Springwood on the Hawkesbury Road Morot Cycle no 04815 on charge to 15AMR driven by NX123915 Cpt V.C McGrath of 15AMR did on 25/01/1943 at 1730hrs collide with a A civilian (Rugby Durant) Sedan No. HZ139 driven by John Vogt of 30 Chister St, Epping causing the death of NX123915 Cpl V.C McGrath. Vehicle M/C has been examined by Court B vehicle no 040815 was an authorised duty. C. Rider was in possession of current licence no 3849 D. Neither vehicle was travelling at excess speed. E The C.O of the unit is not liable under A.M.R a0693 F NY 123915 Cpl McGrath, V.C is not guilty of negligence. G damage as per attached G.4 cost of Inquiry cannot be obtained by this court.
The opinion of the court as to the cause of the accident is that it was due to poor visibility at a blind corner and the driver of the M/C failed to see a vehicle which was being driven from a side road onto a main road which he was travelling on.
