Sergeant Ivan Waddell Storey 408288 RAAF was killed when Miles Master W8369 of Number 53 Operational Training Unit RAF crashed 10 miles west of the village of Westonzoyland in Somerset, England

Sergeant Storey was undergoing advanced pilot training at 53OTU and had force landed Spitfire AR255 at Yeovilton when he ran into difficulties. 

Miles Master W8369 was flown to Yeovilton by Sergeant Herbert Josephs 1387107 RAF to deliver Pilot Officer Basil Wilfred Andrews J/15739 RCAF (of Chicago, Illinois) so he could collect the Spitfire

Sergeant Josephs was then to bring Sergeant Storey back to base in the Miles Master.  The two aircraft were flying in formation on the return journey when they collided in mid-air and all three airmen were killed.
