On page 9 if the Attestation Paper his name is given as John Keating O'Sullivan served and buried as John Gerald O'Sullivan.
Son of Cornelius O'Sullivan and Helena Burke formerly O'Sullivan nee Kelly of Church Street, Newcastle, NSW.
