Upon leaving Australia James was sent to Egypt where he served throughout the duration of his war service with the 11/3rd Light Horse. In 1916 he was regularly in and out of hospital in Egypt receiving treatment for an inner ear condition. James was admitted to hospital at Port Said on 19 May and later in July at the 31st General Hospital before being transferred to the 2nd Australian Stationary Hospital at Tel-el-Kebir on 4 August 1916. He was sent home to Australia on board Ascanius on 1 September with clyvinia otitis media. James was discharged as medically unfit on 18 February 1917.
