Address at the time of enlistment was Monsildale via Lenville, QLD
Son of Hugh Urquhart and Margaret Louisa Urquhart nee McLean of Mumulgum, Richmond River, NSW formerly of Casino, NSW.
Brother of Finley Urquhart who was killed in action at Bullecourt on 11 April 1917 while serving with the 15th Battalion and has no known grave. His name appears on the Villers-Bretonneux Memorial and also Hugh Kenneth Urquhart who returned to Australia on 10 September 1919 having served with the 9th Battalion and Australian Corps Headquarters
16 March 1916 - enlisted at Brisbane, QLD, with the 42nd Battalion and with the rank of Private.
21 December 1916 - transferred to the 41st Battalion in England
31 July 1917 - wounded in action. Severe wound to the left arm and face. Embarked for England on 7 August 1917
16 December 1917 - commenced return to Australia disembarking on 18 February 1918 at Sydney for onward travel to Brisbane

5 May 1918 - discharged at the 1st Military District as wounded
