Son of John O'Flaherty and Ellen E. O'Flaherty nee Stanners of Famwell Street, Croydon, QLD. Later changed to his brother W. O'Flaherty of the Croydon Shire Council, QLD.

Brother of James Augustus O'Flaherty who was killed in action on 27 September 1917 while serving with the 49th Battalion and has no known grave. His name appeares on Menin Gate, Ypres, Panel 23
