Address at the time of enlistment his address was given as Victoria Street, Kurri Kurri, NSW. Year of birth on his Attestation paper calculates as 1889c but the birth record with a mother's name of Agnes gives his year of birth as 1883.

Son of James O'Toole and Agnes O'Toole nee Mills of  Victoria Hotel, Mackay, QLD.

Commenced return to Australia on 19 February 1919 aboard HT Orca disembarking on 3 April 1919
