Special mention in Divisional Orders for acts of courageous gallantry and valuable service  during period 25/4/1915 to 5/6/1915 (Gallipoli)  Not given as a MiD? (see pg 32 of Attestation papers) Mention in Dispatches,  L.G. 29354.

Between 19/2/1917 to 15/9/1917 Lt Hart spent time seconded to the 3rd Light Trench Mortor Battalion and 69th Infantry Battalion before returning to the 12th Infantry Battalion 

Died of wounds suffered at Mt Kemmel France on 25/4/1918 at the 15 Canadian Clearing Station from GSW right thigh fracture and left arm.

Lt Hart had also been wounded 1st on Gallipoli (14/5/1915) but recovered.  He was also wounded (GSW back) in France on 24-25/7/1917 (Controlmaison, Pozieres)
