Son of James O'Toole and Jane Anastasia O'Toole nee Taper of 'Tapersville', Narara, NSW
Brother of Austin Stanley O'Toole who died of wounds on 12 May 1916 while serving with the 20th Battalion and is buried in the Etaples Military Cemetery, Plot V, Row C, Grave 14.
Previously enlisted from 23 March 1915 to 7 April 1915 when he was discharged as medically unfit.
Commenced return to Australia on 27 August 1917 aboard HT Pakeha disembarking on 24 October 1917 at Melbourne then overland to Sydney
