Address at the time of enlistment was given as Railway Coffee Palace, Northam, WA.

Son of Michael O'Keeffe and Margaret O'Keeffe ee Downey of Ballarat, Victoria. 

Brother of James O'Keeffe who was killed in action on 11 October 1917 while serving with the 46th Battalion and has no known grave. His name appears on Menin Gate, Memorial, Ypres, Belgium, Panel 27; Frank O'Keeffe who return to Australia on 8  June 1919 having served with the 46th Battalion

Commenced return to Australia on 10 July 1919 aboard HT Oxfordshire disembarking on 5 August 1919 at Fremantle but failed to re-embark. On 17 August 1919 he re-embarked at Fremantle for onward travel to Melbourne
