Cooke was wounded in the back by a piece of shell on the 5 May 1917 during a heavy bombardment by the Germans. He was evacuated to the 3rd Casualty Clearing Station but died within the day. .
An account in the Red Cross Missing and Wounded file states "He was wounded in the back at Bullecourt and dressed on the spot and taken off by stretcher bearers.... He was a broad, strong chap, 5 foot 10 inches, single, 28, came from Randwick in Sydney, worked at Dalgety's shipping firm, played football for Glebe.
