Full name Ormond Hamilton Maxwell Urquhart

Son of Donald Urquhart and Mary Sibba Urquhart nee Morris of Perth, WA. Brother of Robert Leslie Gibson Urquhart who died of wounds on 3 October 1918 received while serving with the 3rd Division Medium Trench Mortar and is buried in the Netley Military Cemetery, Row N, Grave 409

4 May 1915 -- enlisted at Moora, WA, with the 11th Battalion and with the rank of Private.

1 March 1916 - transferred to the 51st Battalion

16 March 1916 - transferred to the 4th Pioneer Battalion.

8 August 1816 - wounded in action

11 July 1917 - transferred to the 52nd Battalion

20 January 1918 - transferred to the 3rd Division Medium Trench Mortar Battery with the rank of Gunner.

28 February 1919 - commenced return to Australia aboard HT Anchises disembarking on 10 April 1919

Medals: 1914-1915 Star, British War Medal, Victory Medal

3 June 1919 discharged at the 5th Military District
