We HERBERT FURZE of Adelaide in the State of South Australia, Insurance Clerk, formerly for two years on active service abroad with the Australian Imperial Force ARNOLD DAVID FURZE of Port Adelaide in the said State an officer of His Majesty's Customs and MARY OLIVE FURZE the wife of the said Arnold David Furze all being natural born British subjects and all heretofore called or known by the surname of Zschorn hereby give public notice that on the Second day of May One thousand nine hundred and Twenty-one each of us severally and formally renounced the use of the said surname Zschorn and then assumed adopted and thenceforth determined on all occasions whatsoever to use and subscribe the surname of Furze instead of the surname Zschorn.

Dated this Second day of May One thousand nine hundred and twenty-one.
