Son of James Arthur and Jessie O'Sullivan, of Balwyn, Victoria.
The accident resulting in the death of LAC O'Sullivan occurred in the guard room at No. 7 S.F.T.S. Deniliquin at 2025 hours on the 2nd day of January 1944. O'Sullivan who was serving a sentence of Field Punishment, entered the guard house with other prisoners and stood to the left of LAC Thomas John Walsh, guard of No. 7 S.F.T.S who was in the process of endeavouring to remove five rounds from a Smith & Wesson .38. While Walsh was handling the pistol one round was discharged, the bullet entering O'Sullivan's right lower abdomen. O'Sullivan was place in charge of the S.M.O. within 30 seconds, he being at that time outside the guard house. After bening treated for shock at Station Sick Quarters, O'Sullivan was removed to the Deiliquin Public Hopsital at 2215 hours 2nd January 1944 and operated on by S/L J.C. Sangster, the Station S.M.O. Later he was given a blook transfusion. At 1310 hours 3rd January 1944, O'Sullivan died.
