On the 12th of February 1941 Wirraway A20-121 collided head on with A20-138 on a runway at Amberley Aerodrome - both aircraft burnt and totally destroyed. Cause listed as carelessness and lack of vigilance as the aircraft taking off struck another aircraft taxying back in head on. Aircraft and crew were part of Number 3 Service Flying Training School
Flying Officer Patrick Adair Black 558 and LAC Raymond Alexander Humphry 406250 were killed in aircraft A20-121 and they are commemorated in Ipswich Cemetery, Queensland
Flight Sergeant Nigel Watson 1945 and LAC Douglas Porter Scott 406229 were killed in aircraft A20-138 and they are commemorated on the Queensland Cremation Memorial in Lutwyche Cemetery, Brisbane (LAC Scott was originally listed as injured but died the same day)
(NB: Wirraway A20-121 is incorrectly listed as A20-201 on some records - that aircraft was destroyed in a separate incident on 19 January 1941)
