Son of Thomas Urquhart and Mary Jane Urquhart nee Perry of 'Raby', Coleraine, Victoria

7 September 1916 - enlisted at Melbourne, Victoria, with the 5th Battalion and with the rank of Private.

27 August 1917 - transferred to the 21st Battalion. Re-allotted service number now 6827A
