Thomas was admitted to Harefield Hospital, showing signs of "melancholia" on 08 Feb 1918. After his transfer on 19 Feb 1918 to Middlesex County Hospital, Hapsbury he was reported to be "dull, listless, apathetic, spends most of time huddled on floor…becoming rapidly demented."

By 16 Mar 1918, Thomas' diagnosis was listed as "Dementia Praecox." His disability was attributed to service due to "stress of campaign" and classed as 100% disabled.

Medical Board at Hapsbury recommended he be "discharged to asylum" which on 16 Jul 1918 was approved by the Director of AIF Medical Services.

Thomas returned to Australia per HMAT Boonah on 17 Sep 1918 

At Launceston Hospital on 01 Jul 1919 it was noted "condition of mental illness still present, general health good".

Thomas drowned in floodwaters at Reedy Creek on 07 Nov 1924, which was rules by the Coroner as accidental.
