On 4 September 1916 Thomas Richard Carroll was killed in action at Mouquet Farm in the Pozieres area of France, while serving with the 52nd Battalion. Creating confusion to all was the fact his cousin 4470 Richard Thomas Carroll, who enlisted with him from Irishtown in Tasmania and was serving in the same unit, was also killed in action on the same day at Mouquet Farm.

Thomas Richard Carroll was born in 1892 and his mother died just after his birth. He had 8 brothers and sisters, one of them, 3275 Pte. Harry Michael Carroll 12th Battalion AIF, was killed in action about a fortnight before at Pozieres, 19 August 1916. 

Thomas was listed as missing for a long time until confirmed as killed in action. His remains were discovered by the Graves Unit in late 1923 and reinterred in the Bouzincourt Ridge Cemetery. His two recovered identity discs were sent back home to his sister in 1924.
