The following statement is taken from the service record of John James Abberton found on naa.gov.au

Date of origin of disability: July 3 1916
Place of origin of disability: France
Give concisely the essential facts of the history of the disability, noting entries on the Medical History Sheet bearing on the case: Fell from his horse on to his right shoulder which later became the seah of Rheumatism for which he was admitted to Harefield on September 22nd.
On Octiber 8th Rheumatism invoved both hips and ankles and the left knee had Rheumatism 3 1/2 years ago
Give your opinion as to the causation of the disability: Active service
If you consider it to have been caused by active service, climate, or ordinary military service, explain the specific conditions to which you attribute it: Training & Climatic
What is his present condition: Pain in right shoulder. Both hips and ankles. Left knee and both ankles definitely involved.
Do you recommend discharge as permanently unfit, or change to England: Discharge
