Theophilus was born in Ulverstone, Tasmania on 4th May, 1898

His parents were William HAYES & Florence Louisa CORNICK - his mother Florence died on 17th May, 1898 only 13 days after he was born

He first enlisted on 28th January, 1915 and embarked with the 12th Infantry Battalion, 5th reinforcements on the HMAT Hororato on 17th April, 1915 from Melbourne - he was returned to Australia due to illness and was discharged on 6th August, 1915

He enlisted again on 21st July, 1916 and embarked with the 40th Infantry Battalion, 2nd reinforcements on the HMAT Ballarat from Hobart on 8th August, 1916

He died of wounds in France on 28th March, 1918 and is buried in the Querrieu British Cemetery and his name is memorialised on the Australian War Memorial

His brother Elvin Hansell Raymond HAYES (SN1861) was Killed in Action on 12th February, 1917
