"Frank Percival Johnson, born Laura, South Australia, 8 May 1891. Enlisted at Oaklands, South Australia, 30 November 1914. Served Gallipoli and France. 3rd Field Ambulance. Awarded Military Medal (MM) for bravery in the field and the Meritorious Service Medal (MSM) in recognition of valuable services rendered with the Armies in France and Flanders.
