Son of Frederick William Ullathorne and Lavinia Ullathorne nee Pountin of Kent Street, Windsor, Vicroria.

Husband of Annie Ullathorne of High Street, Runcorn, Cheshire, England. Guy and Annie were married on 2 March 1917 in Runcorn, Cheshire, England. Rev. J.E. Howard officiated.

Commenced return to Australia on 7 June 1918 aboard HT Essex disembarking on 1 August 1918 at Sydney.
