Herbert John Cook was born in Payneham, South Australian on 12th of October 1881. Prior to the war, Herbert worked as a Wheelwright. He lived at Kent town with his wife and kids. Herbert was married to Amelia Mathilda (Milde) Cook and together they had 3 children. Herbert identified as a member of the Church of England.

Herbert enlisted in Adelaide at the age of 33. He departed on the 4th of October 1915 aboard the HMAT A47 “Mashobra”, joining the Australian Army Medical Corps. He disembarked at Alexandria and then proceeded to Gallipoli serving with the No. 3 Field Ambulance, during the final days of Gallipoli. He was then transported back to Alexandria. He was hospitalized with Rheumatism on 9th of January 1916. He was later transferred to a ward for medical diseases, and by March 1916, Herbert was fit enough to join the 2nd Field Ambulance.

His service continued on the Western Front with hospitalizations in 1916 for dental issues and diarrhoea. He was appointed as Lance Corporal in November 1916.

The following year on March 1st 1917 Herbert was wounded in action and transfered to the hospital with a gunshot wound to his back listed by one account as “mild”.  He was sent to England for further tratment and convalescence before returning to France in October 1917, serving again with the 2nd Field Ambulance. In December 1917, Herbert was diagnosed with Trench Fever and sent back to England. After further medical assessment, he was discharged to Australia in April 1918 aboard H.M.T. A74 due to chronic catarrhal otitis media (a persistent middle ear infection).

Herbert died aged 74 on 31 May 1956.
