Son of Michael O'Keeffe and Margaret O'Keeffe ee Downey of Ballarat, Victoria. 

Brother of James O'Keeffe who was killed in action on 11 October 1917 while serving with the 46th Battalion and has no known grave. His name appears on Menin Gate, Memorial, Ypres, Belgium, Panel 27. Mark O'Keeffe who returned on Australia on 13 August 1919 having served with the 3rd Light Horse Brigade Machine Gun Squadron.

Commenced return to Australia on 13 April 1919 aboard HT Commonwealth disembarking on 8 June 1919
